【右丁】
一 同四【三】午年四月、御部屋において長昌院の方が御臨産につき、朔日から昼夜詰切って相勤めたところ、同六日の夜に宣明院様が御誕生なされた。これにより御七夜の節、御誕生につき昼夜精を出して相勤めたことに対し、御褒美として金二百疋を下し置かれた事。
一 同月、右の件につき六日の夜中に愿彰院様の御前へ召し出され、御懇切な御意を蒙った事。

一 同年九月、愿彰院様が御俳諧をなされた節、御席へ召し出され、この後御参日の毎度罷り出る事。
一 同五【四】未年二月、病用出精いたし、その上愿彰院様の御診を懈怠なく精出して相勤めた段、寄《特》に思し召され、これにより三人扶持の御加増を下し置かれる旨仰せ出された事。

- In the 4th [3rd] year of the Horse, 4th month, when Chōshōin-no-kata was in labor in the御部屋 (private chambers), [the physician] served continuously day and night from the 1st day. On the night of the 6th day, Senmyōin-sama was born. For this, during the seventh-night celebration, in recognition of diligent service day and night for the birth, he was awarded 200 hiki of gold as a reward.
- In the same month, regarding the above matter, on the night of the 6th day, he was summoned before Genshōin-sama and received gracious words.

- In the 9th month of the same year, when Genshōin-sama composed haikai poetry, he was summoned to the gathering and thereafter attended every ceremonial day.
- In the 5th [4th] year of the Sheep, 2nd month, he devoted himself to medical duties, and moreover served diligently without negligence in examining Genshōin-sama. This was particularly appreciated, and therefore he was granted an additional stipend of three persons' worth.
