中山延柳と申医師平兵衛同道ニ而福井江着御泉水御屋敷江
罷出 御脈相伺之 御病気御太切至極ニ付御薬指上候儀御断
申上候得共
吉邦様強而御願被成候ニ付御薬指上之
但翌十一日 御脈再相伺候処弥御太切ニ付御薬難指上候間
御暇被下候様申上候得共御残念 思召候間今一貼成共

一 九月六日、病気見舞いの礼のため、御使者寄合の本多小助（良正）を江戸表へ派遣した。今日福井を出発。
一 同月十日、御使番出渕平兵衛（盛房）を通じて呼び寄せた京都の医師両人とも体調不良のため、中山延柳という医師が平兵衛と同道して福井に到着し、御泉水御屋敷に参上して脈診を行った。病状が極めて重篤なため、薬を処方することをお断り申し上げたが、吉邦様が強くお願いされたため薬を処方した。
ただし翌十一日に再び脈診したところ、いよいよ重篤なため薬を処方することは困難であり、お暇をいただきたいと申し上げたが、残念にお思いになられたので、もう一服でも薬を処方した上でお暇をいただきたいと仰せ出されたため、薬を処方して直ちに京都へ帰った。
一 同月十二日未刻（午後2時頃）、御泉水御屋敷において逝去された（享年七十二歳）。
一 右の件につき、公儀へ報告し、かつ御一門様方への知らせのため御使者として、御側物頭大野三左衛門（正矩）、御使番樋口小左衛門（安隆？）を江戸表へ派遣した。今日福井を出発。
一 同月十四日、御遺言により瑞源寺への葬送を希望される旨を昨十三日に運正寺へ申し出たところ、御家の宗門の事であればその件はお受けできないと回答があったため、御奏者番嶋田宇右衛門（重休）、寺社町奉行渥美［以下未完］

1. On September 6th, as a gesture of gratitude for the inquiry about his illness, an envoy, Yoriai Honda Kosuke (Yoshimasa), was dispatched to Edo. He departed from Fukui today.

On the 10th of the same month, both Kyoto physicians who had been summoned through Messenger Debuchi Heibei (Morifusa) were unwell, so a physician named Nakayama Enryu accompanied Heibei and arrived in Fukui, visiting the Sensui residence to examine the pulse. Due to the extremely critical condition of the illness, he declined to prescribe medicine, but since Lord Yoshikuni earnestly requested it, he prescribed medicine.
However, when he examined the pulse again on the following 11th day, the condition was even more critical, making it difficult to prescribe medicine, so he requested to take his leave. But since His Lordship felt regretful, he was ordered to prescribe at least one more dose of medicine before being granted leave, so he prescribed medicine and immediately returned to Kyoto.
1. On the 12th of the same month at the hour of the Sheep (around 2 PM), he passed away at the Sensui residence (at age 72).
