種痘致施行度（しゆとうせきよにいたしたき）医師（いし）は旧東校（もととうこう）迄申
出 詮議（せんぎ）之上 免状（めんじよふ）相渡来候処各府
県下に於て右免状相受候医師不少候間
自今は免許（めんきよ）相請候医家ゟ其 術（じつ）習（しゆ）
練（れん）之旨弟子之 管轄庁（ところやくしよ）に申出候はゝ地
方官限詮議を遂不都合之次第
無之候はゝ施行免許致すへき事

現代語訳

種痘を実施したい医師は、旧東校まで申し出て、詮議の上、免状を交付してきましたが、各府県下において右の免状を受けた医師が少なくない状況です。今後は、免許を受けた医家からその術を習練した旨を弟子の管轄庁に申し出れば、地方官限りで詮議を行い、不都合な事情がなければ施行免許を与えるべきこと。

英語訳

Physicians who wish to perform smallpox vaccination should apply to the former Tōkō (Eastern School), undergo examination, and receive a license. However, there are now many physicians in various prefectures who have received such licenses. From now on, when disciples report to their jurisdictional office that they have learned and practiced the technique from a licensed physician, local officials shall conduct examinations within their authority and grant practice licenses if there are no unsuitable circumstances.
