現代語訳
御目見えを仰せ付けられる旨を仰せ出され、御在国の毎度正月二日の御規式において御礼を申し上げ罷り在り、御違例（御病気）の時は召され拝診仕り候
一、享保二十年（1735年）卯年三月、二十人扶持金二枚を下し置かれ、常詰御医師に召し出され、御保養の筋を津田宗庵へ申し談じ相勤むべき旨を仰せ出され、同四月十六日発足、同二十二日まで着仕り候ところ、即日御座之間において御目見え、直ちに拝診を仰せ付けられ、同年の冬より御薬をも差し上げ候事
一、同月、愿彰院様の御診を始めて拝診を仰せ付けられ候事
一、同年五月、御納戸払いの御小袖・御帷子を下し置かれ、この後御納戸払いの毎度同様に下し置かれ候事
一、同年九月、御家中の療治を多く致し候につき、薬種代一年金五両宛を下し置かれ候

He was granted an audience with the lord, and during each New Year's Day ceremony when the lord was in residence in the domain, he would pay his respects and remain in attendance. When the lord was ill, he would be summoned to provide medical examination.
1. In the 20th year of Kyōhō (1735), the year of the Rabbit, in the third month, he was granted a stipend of twenty men's portions and two gold pieces, and was appointed as a regular court physician. He was ordered to consult with Tsuda Sōan regarding the lord's health care and to serve accordingly. He departed on April 16th of the same year and arrived by the 22nd. On that very day, he had an audience in the formal reception room and was immediately ordered to conduct a medical examination. From the winter of that same year, he also began preparing medicines.
1. In the same month, he was ordered to conduct his first medical examination of Lord Ganshōin.
1. In the fifth month of the same year, he was granted used court robes and undergarments from the lord's wardrobe, and thereafter was regularly granted similar items from the lord's wardrobe.
1. In the ninth month of the same year, because he provided extensive medical treatment to members of the lord's retainer group, he was granted an annual allowance of five gold pieces for medicinal ingredients.
