一宝暦三酉年願之通御擬作被 召上
銀子三枚被下候
但勤中数度御褒美被下江戸勤番
度々相勤其毎度銀子壱枚宛被下候由之

目付平出久兵衛相詰と在之元文五年
十一月七日御賄頭平出久兵衛無懈怠精
出宜相勤候ニ付弐石壱人扶持被下候
趣相見本文と齟齬致候ニ付参考之為記置候

病死法名秋嶽徹霜居士葬所同上

一同年十二月 御小袖と御上下の代金を下賜されたこと
一《年月不詳》 五石の加増を下賜されたこと
一宝暦三年（酉年） 願いの通り御擬作職を召し上げられ、銀子三枚を下賜された
ただし勤務中に数度褒美を下賜され、江戸勤番を度々勤め、その都度銀子一枚ずつを下賜されたとのことであるが、詳細は不明である
一享保七年（寅年）十二月二十三日、同二十五日に御徒目付平出久兵衛が詰めていたとあり、元文五年十一月七日に御賄頭平出久兵衛が怠りなく精励してよく勤めたため、二石一人扶持を下賜されたという趣旨が見られるが、本文と食い違うため参考のため記しておく
一明和元年（申年）九月十三日 七十一歳で病死、法名は秋嶽徹霜居士、葬所は同上

Note: He received commendations several times during his service and frequently served in Edo duty, receiving 1 silver piece each time, though details are unclear
- Kyōhō 7 (Year of the Tiger), 12th month, 23rd day: On the same 25th day, there is a record of ashigaru metsuke Hiraide Kyūbei being on duty, and on Genbun 5, 11th month, 7th day, osewagashira Hiraide Kyūbei diligently performed his duties without negligence and was granted 2 koku and support for 1 person. This account differs from the main text, so it is recorded here for reference
- Meiwa 1 (Year of the Monkey), 9th month, 13th day: Died of illness at age 71, posthumous Buddhist name Shūgaku Tesshō Koji, burial site same as above
