御重恩之万ケ一茂度常々念願罷在候処従先月
中持病之疝積并胸痛強差発難儀仕候ニ付
種々療養仕候得共果敢取不申一体相催候
病疾故早速快方可相成休無御座候間御用多
之御役儀抱罷在永々不勤仕候様深恐入
心配仕候依之未引込間茂無御座奉願候儀
是亦恐入奉存候得共何度此上之以
御慈悲両御役儀被成下 御赦免候様奉
願候此段御年寄中迄宜様被仰上可被下候

現代語訳
御重恩の万が一でも報いたいと常々願っておりましたところ、先月中より持病の疝積（せんしゃく）ならびに胸痛が強く発症し、困っております。種々療養いたしましたが、効果が上がらず、一体に相催す病気であるため、早速快方に向かう見込みがございません。御用の多い御役儀を抱えたまま長々と勤務できない様子で、深く恐れ入り心配いたしております。
これにより、まだ引き込み（隠居）の間もございませんのに願い出る儀は、これまた恐れ入ると存じますが、何卒この上の御慈悲をもって両御役儀をお免じ下さいますよう願い奉ります。この段、御年寄中まで宜しく仰せ上げ下さいますようお願いいたします。

英語訳
I have always wished to repay even a fraction of your great benevolence, but since last month, my chronic illness of intestinal obstruction and chest pain have severely worsened, causing me great difficulty. Although I have tried various treatments, they have not been effective, and since this is a condition that tends to recur, there is no prospect of quick recovery. I am deeply troubled and worried that I cannot continue to perform my duties for long while holding these important official positions with many responsibilities.
Therefore, although it is also presumptuous of me to make this request when I have not yet reached the time for retirement, I humbly beseech your mercy to grant me exemption from both official positions. I request that you kindly convey this matter to the council of elders.
