一、養子の方については、その実父のお名前・実名ともに

一、亡くなられた方については、年月・法名・年齢ともに

Beginning with your ancestor Koyanagawa Nakatsukasa-tayū Morimune and continuing to the present, please compile and submit a written record — in proper order — of all members of the main lineage as well as collateral lines (brothers and sisters) across all generations, both male and female, covering the following items:
1. Both common name (俗名) and formal personal name (実名)
2. For adopted sons: the name and formal personal name of their biological father
3. Among siblings: those who succeeded other families, as well as daughters who married into other families
4. For the deceased: year and month of death, posthumous Buddhist name (法名), and age at death
As the above information is required, I hereby make this request. Even if the record is not complete in all respects, please submit whatever is known. That is all.
