但本末之由緒委細可書載之由緒無之ハ由緒無之与可認
一最前之宗旨ニ相改候ハ以前ハ何宗何年ゟ当宗ニ改派之段可相認之
一安置之本尊何開山誰何ノ年開基
但開基之寺地ゟ所替候ハヽ以前之寺地ハ何方与可認之
一寺格委細之由緒 綸旨紫衣地等之訳を初可認之
一寺地間数并建物 本堂何間ニ何間対面所同断庫裏同断縁ハ准右
大鐘并鐘楼之有無
一塔頭何ケ寺 寺号
但当時建物無之ハ其訳可相認之
一境内之外ニ有之致支配来候堂舎并下屋敷隠居所或ハ廃寺之跡地等之
地面間数当時建物之有無
但何ノ年ゟ致支配候訳
一御朱印地除地等之由来
一持高持山等之由来
一御寄附知行高并由来
一葵御紋冪類御寄附所持之由来
