一享保元申年三月親願之通御番
代被 仰付神保茂左衛門組ニ而
御城御番相勤候事
一同四亥年十一月跡式御知行百石無
御相違被下置候事
一同年七月以神保茂左衛門願之通隠
居ニ付跡御役杉田五兵衛組ニ罷成候事
一同六丑年五月御供番被 仰付候事
一同月宇左衛門ニ改名仕度旨被伺願候

現代語訳
一、享保元申年三月 親の願いの通り御番代を仰せ付けられ、神保茂左衛門組にて御城御番を相勤めた事
一、同四亥年十一月 跡式御知行百石、御相違なく下し置かれた事
一、同年七月 神保茂左衛門の願いにより隠居に付き、跡役として杉田五兵衛組に罷り成った事
一、同六丑年五月 御供番を仰せ付けられた事
一、同月 宇左衛門に改名仕りたき旨を伺い願った事
一、同七寅年四月 勤番として江戸表へ罷り登り、同十月に罷り下った事
一、同九辰年十月 江戸へ勤番として罷り登り、翌巳五月に御供にて罷り下った
但し御道中御戸番仮役を相勤め罷り下った事
一、同十四酉年五月 御戸番を仰せ付けられ
一、同十五戌年四月 御供にて勤番罷り登り、同十月罷り下った

1. Kyōhō 1 (1716), 3rd month - As his father requested, he was appointed as replacement guard and served castle guard duty in Jimbo Mozaemon's unit
1. Kyōhō 4 (1719), 11th month - He was granted the hereditary stipend of 100 koku without any changes
1. Same year, 7th month - Due to Jimbo Mozaemon's request for retirement, he was transferred to Sugita Gohei's unit as his successor
1. Kyōhō 6 (1721), 5th month - He was appointed as attendant guard (gosuppoban)
1. Same month - He petitioned to change his name to Uzaemon
1. Kyōhō 7 (1722), 4th month - He went up to Edo for guard duty service and returned in the 10th month of the same year
1. Kyōhō 9 (1724), 10th month - He went up to Edo for guard duty and returned in the 5th month of the following Snake year as part of the lord's retinue
Note: He served as temporary gate guard during the journey back
1. Kyōhō 14 (1729), 5th month - He was appointed as gate guard (gotoba-ban)
1. Kyōhō 15 (1730), 4th month - He went up for guard duty as part of the lord's retinue and returned in the 10th month
Note: Upon his return, he was granted used formal court dress (kamishimo)
