ニ付御供仕御新葬之節ともニ御用相勤其後
西郷頼母日向三郎右衛門中沢六右衛門一同
代り番ニ廿日ツヽ見祢山江相勤候事
一 同二寅年五月老年之儀格別ニ於会津
乗物御免被成之旨被 仰出候事
一 同三卯年八月見祢山 御社御遷座ニ付
御用被 仰付候右ニ付吉川惟足被罷下候ニ付御馳
走之儀肝煎候様被 仰付猪苗代江一同往来

一 同年九月御銕炮ニ而被為留候鶴之御料理

現代語訳
そのためお供して、御新葬の節ともにご用を勤め、その後は西郷頼母、日向三郎右衛門、中沢六右衛門と一同で代り番に二十日ずつ見祢山へ勤めに行った。
一 同二寅年五月、老年のことを格別に配慮され、会津において乗物をお許しになる旨を仰せ出された。
一 同三卯年八月、見祢山の御社の御遷座につきご用を仰せ付けられた。このことにつき吉川惟足が下向したため、御馳走の件を取り仕切るよう仰せ付けられ、猪苗代へ一同で往来した。
一 同年九月、御鉄砲で撃ち留められた鶴の御料理を下された。
一 同四辰年正月四日、六十八歳で病死。霊号は貞彦霊社。葬所は大窪山。
ただし、かねてより吉川惟足から神道を習得し相伝を受けていたため、大窪山に葬地を申請しておき、没後は神道をもって葬祭を行い、霊号も惟足から贈られた。
女 竹 《慶長十五戌年誕生、貞享二丑年七月二十六日江戸において七十六歳で病死。法名は仏寿院妙縁。後に神道に改めて祭り、霊号は寿姫神霊。葬所は不詳》

Therefore, he accompanied [the lord] and served during the new funeral ceremonies. Afterward, he took turns with Saigō Tanomo, Hyūga Saburōemon, and Nakazawa Rokuemon, serving at Mount Mine for twenty days each.
1. In the fifth month of the second year of the Tiger (Enpō 2, 1674), in special consideration of his advanced age, he was granted permission to use a palanquin within Aizu domain.
1. In the eighth month of the third year of the Rabbit (Enpō 3, 1675), he was assigned duties in connection with the relocation ceremony of the shrine at Mount Mine. For this occasion, as Yoshikawa Koretari came down from Edo, he was ordered to arrange the hospitality, and they all traveled back and forth to Inawashiro.
1. In the ninth month of the same year, he was granted prepared crane that had been shot with the lord's rifle.
1. On the fourth day of the first month of the fourth year of the Dragon (Enpō 4, 1676), he died of illness at age sixty-eight. His posthumous spiritual name was Sadahiko Reisha, and he was buried at Mount Ōkubo.
However, since he had previously learned and received transmission of Shinto practices from Yoshikawa Koretari, he had requested a burial site at Mount Ōkubo in advance. After his death, the funeral rites were conducted according to Shinto practices, and his spiritual name was also bestowed by Koretari.
Daughter: Take 《Born in Keichō 15 (1610), died of illness in Edo on the 26th day of the 7th month of Jōkyō 2 (1685) at age 76. Buddhist name: Busshū-in Myōen. Later re-consecrated according to Shinto practices with the spiritual name Suhime Shinrei. Burial site unknown.》
