是ノ天明三年信州浅間山焼出シ上州山
津浪ニ付近国ヨリ御届書並ニ諸訴書
ハ茨城県猿島郡古河町大字悪戸新田
小堀豊吉氏ノ所蔵ニ係ルヲ借用シ来リ
是レヲ録事補ニ耕写セシメテ永久ニ保管
シ後年ノ参考ニ資セント云フ

現代語訳
この天明三年信州浅間山噴火による上州山津波に関する近隣諸国からの届出書並びに各種訴状は、茨城県猿島郡古河町大字悪戸新田の小堀豊吉氏の所蔵によるものを借用して持参し、これを録事補に書き写させて永久に保管し、後年の参考に資するためのものである。

英語訳
These official reports and various petitions from neighboring provinces regarding the Shinshū Mount Asama eruption and the resulting Jōshū landslide tsunami in Tenmei 3 are borrowed from the collection of Mr. Kobori Toyokichi of Akuto Shinden, Koga-machi, Sashima-gun, Ibaraki Prefecture. They have been transcribed by an assistant clerk for permanent preservation and to serve as reference for future years.
