むやみに様々な丸薬・散薬を用いることはしてはならない。ただし、熊の胆（くまのい）は用いても差し支えない。
肥前国天草郡の民は、痘瘡（天然痘）を患うことがない。そのため、痘瘡を恐れること、蛇や蠍（さそり）よりもはなはだしい。万一、罹患した者がいる時には、親子・一族の縁を断ち切って、病人を山野に捨ててしまう。また、近隣の郡で痘瘡が流行すると、大勢が集まって鬨の声を上げ、鉦や太鼓を鳴らし、鉄砲を放ってこれを追い払おうとする。隣国に痘瘡の

患者があれば、船を止めて通路を断ち切る。このように痘瘡を避けて罹患しない地域が、日本各地に存在する。これを見れば、（痘瘡は）避けようとしても避けられないものでもないことがわかる。
私（著者）が思うに、流行する疫気（えきき・病気の気）を避けることができれば、父母から受け継いだ胎毒（遺毒）も発動することはない。また、遺毒を解消しておけば、疫気もこれを犯すことはない。よくよく見るべきことは、あらかじめ防ぐべきものは痘瘡である、ということだ。

In my view, if one avoids the epidemic miasma (*ekiki*) when it spreads, even the latent poison (*idoku*) inherited from one's parents will not be stirred into action. And if the latent poison is resolved in advance, the epidemic miasma will not be able to invade. It should be clearly understood: smallpox is above all a disease that must be prevented beforehand.
