現代語訳
病気についてお尋ねいただき、ご懇切なお言葉をいただいた上で、養生の件について思いつくことがあれば遠慮なく申し上げるようにとのご命令をいただいた。
ただし、右の件について思うところを書き記して差し上げた。
一 寛永元辰年二月 お年を重ねられたお祝いとして、ご懇切なお言葉をいただいた上で真鴨一羽を賜った。
一 同年七月四日 六十四歳で病死。法名は安清院浄誉杏庵居士。葬所は盛道山高巌寺。
（没後七月十三日、杏庵は実直な人物であったところ、不憫にお思いになられたとのご意向を津田宗庵が申し聞かせた）

He was inquired about his illness and given kind words, then commanded to speak without reserve about anything he might think of regarding recuperation matters.
However, he submitted in writing his thoughts on the above matter.
One: In the 2nd month of Kan'ei 1 (1624), Year of the Dragon - As celebration for his advancing years, he was given kind words and granted one mallard duck.
One: 4th day of the 7th month of the same year - Died of illness at age 64. Buddhist name: Anseiin Jōyo Kyōan Koji. Burial place: Seidōzan Kōganji Temple.
(On the 13th day of the 7th month after his death, Kyōan was an honest person, and [the lord] expressed that he felt sorry for him, as conveyed by Tsuda Sōan)
