のあつまりし所（ところ）は出痘（ほうさう）も稠密（おほくてる）ものにて後（のち）かならす痒瘙（かゆみ）を発（はつ）す

痘中脈を診さる説
張寅賔（てういんひん）曰（いはく）人（ひと）の病（やまひ）は脈（みやく）をもて其（その）病源（やまひのもと）を詳（つまひ）らかにすれど惟（たゝ）痘の一症（いつしやう）は
他（ほか）の病（やまひ）とちかひて内（うち）より興（おこ）りて外（そと）に発（はつ）し一二日／発（はつ）し尽（つく）せは三四
日／肉腫（からたはれ）て五六日山をあげ次第（したい）に膿（うみ）をもよふし痂落（かせ）の時（とき）なとは稠密（てものおほき）
ものなれは老松皮（ふるまつかわ）の如（こと）く或（あるひ）は鶏糞（にはとりのふんを）殻（かけたる）如（こと）くにて功（こう）を収（おさめ）るなりされは
出瘡（てもの）のうへより脈を診（うかゝ）ふたれはとて綿入（わたいれ）布子（ぬのこ）のうへより脈を切（み）る

【右丁】
毒が集まった所は出痘も密集して多く現れるもので、後に必ずかゆみを発するものである。庸医や一般の家庭では、その理由を知らずに、むやみに按摩することを良いとしている。今後は慎んで禁止すべきである。
痘瘡中に脈診をしない説
張寅賓が言うには、人の病気は脈によってその病気の原因を詳しく知ることができるが、ただ痘瘡の一症だけは他の病気と違って、体内から起こって外に現れ、一、二日で発疹し尽くせば三、四日で肉が腫れ、五、六日で山のように盛り上がり、次第に膿を持つようになり、かさぶたが落ちる時などは発疹が密集したものであれば、古い松の皮のようになり、あるいは鶏糞の殻をかけたようになって治癒するのである。そのため出疹の上から脈を診ようとするのは、綿入れの布子の上から脈を診るのに似ている。それ故に昔の人も、痘瘡中に脈を診うのは靴を隔てて痒いところを掻くようなものだと言っている。

剝兎のようだと言って、出疹の皮が破れて膿血が流れて、兎の皮を剥いだような状態になることがある。また収痂することが厚くて乾いた形がミミズの灰にまみれたようになることがある。あるいは爛痘と言って、かさぶたがめくれ上がって膿血が爛れて流れることがある。これらの症状に対して脈を診ようとしても、その症状を詳しく知ることはできないであろう。そのうえ痘瘡は陰の病が陽に発するものゆえ、その脈は必ずしも陽脈を現すものである。もし脈によって治療を施さなければ、人の命を害することは少ないであろう。この故に私の家の術では、唇・舌と形・色とを第一として、その日期を推察し、内外の症状を詳らかにして治療の規則とする。痘瘡に対して脈を論ずる者たちは、その精要の妙理を悟っていないのである。しかし熱の時と痂収の後とは脈によるべきである。

Theory Against Taking the Pulse During Smallpox
Zhang Yinbin said: Human diseases can be diagnosed in detail through examining the pulse to understand their source, but smallpox alone differs from other diseases in that it arises from within and manifests externally. If the eruptions appear completely in one or two days, then on the third or fourth day the flesh swells, on the fifth or sixth day it rises like mountains, gradually forming pus, and when the scabs fall off, if the eruptions are dense, they become like old pine bark or like scattered chicken droppings before healing. Therefore, trying to take the pulse over the eruptions is like taking the pulse through a padded cloth jacket. For this reason, ancient physicians said that taking the pulse during smallpox is like scratching an itch through one's shoes.

There is what is called "peeled rabbit" - when the skin of the eruptions breaks and pus and blood flow, resembling a skinned rabbit. There are also cases where the scabs form thickly and when dry resemble earthworms covered in ash. Or there is "festering smallpox" where the scabs curl up and pus and blood fester and flow. Even if one attempts to take the pulse in the face of these symptoms, it would be impossible to understand the symptoms in detail. Moreover, since smallpox is a yin disease manifesting as yang, its pulse necessarily shows a yang pulse. If treatment is not administered based on the pulse, harm to human life would be minimal. For this reason, in my family's medical practice, we consider the lips, tongue, form, and color as primary, infer the timing, clarify internal and external symptoms, and use these as the rules for treatment. Those who discuss the pulse in relation to smallpox have not grasped the essential subtle principles. However, during fever and after scab formation, the pulse should be consulted.
