便通について：全く便通がない場合もあるが、むやみに下剤を使ってはいけない。腕の悪い医者や一般の家庭などが、むやみに下剤を用いて、一律に毒気を下せばよいことだと心得ているのは愚かなことである。そもそも痘瘡には気虚から始まるものもあり、内虚から始まるものもあり、表虚から始まるものもある。また実熱から始まるものもあり、表実から始まるものもあり、内実から始まるものもある。その症状に応じて下すべきものは下し、補うべきものは補うべきである。むやみに下剤を用いて虚症の者に与えれば、人を殺す恐れがあるので注意すべきである。
一、前にも記したように、痘瘡の疎密軽重（まばらか密集か、軽いか重いか）を問わず、稀痘解毒の妙方・奇方といっても、むやみに用いてはならず、十分に慎むべきである。また生冷のもの（冷たいもの）を与えてはならず、何でもよく煮熟したものを飲食させるべきである。
一、初熱が二三日程度で痘が初めて出るのを見点という（俗に「ほみせ」という）。また三四日で足の裏に痘が出るのを出斉という（俗に「てそろい」という）。

初めに出た痘がまず肥大し光沢があって、後に出た痘も次第に肥大していくのがよいとする。この時小便が度々通じるのは最もよいことである。
一、もし初めの二三日のうちに熱が甚だしく、口が渇き、声が枯れて大便が通じず、小便も渋り、あるいは通じても血のようなものがあり、あるいは空嘔吐が頻繁にあるものもある。目の中が甚だしく赤く、血を注ぐようで朱を塗ったようになり、顔色も赤く、あるいは青いものもある。あるいは昼夜うわごとを言って騒がしく狂妄のようになるものもあり、また腰腹を痛がって眠ることができず、あるいは全身の発疹が赤紫で、甚だしいものは焦げて黒くなるものもある。急によい薬を用いて大便を下すのがよい。もし治療が遅れると救い難い。
