一、出斉（てそろい）から三日間を起脹（きちょう）という（俗に「山開け」という）。大体初めから四日に至って、全身の熱が冷めて元気がよく、飲食が進まなくても普段のようによく眠り、声も枯れずに、小児であれば平常好む玩具などを取り寄せて遊ぶのがよい。しかし気難しい性格のため、遊び戯れる心はあるものの、あるいは投げ捨てて我がままを言って寝たり起きたりなどするのは、熱の後であるため心が爽やかでないからである。この時腹下りが止んで小便がたくさん度々通じるのがよい。またこの時くしゃみが度々出るものである。数多く出るほどよい。痘瘡の理を理解していない者は外感風邪だと心得て治療を誤るものである。心得るべきである。
一、五六日で痘瘡が盛んに肥大し、桃紅色に光って、顔も浮腫み、次第に眼瞼も腫れて目が開きにくくなるのはよろしい。また手で痘瘡を撫でてみれば

皮が厚く水漿が漏れず、手で珠を撫でるようで、しかも艶の潤いがあるのは最も大吉である。この時よく眠り、よく食べ、声清らかで大便が硬く小便が通じるのを第一によろしいとする。これが五六日の験である。もし七日の期に至って稠密（発疹が多い）なもので、この時一度大便が通じれば殊の外悪い。特に大人の痘症で、この時少しでも大便が通じる時は、たちまち順調なものも逆転するものである。すべて大人小児ともにこの期において大便が通じる者は甚だ重大であると知るべきである。速やかに治療しなければ必ず凶逆に変化する。
