一、正月十六日、町右同様の状況であったが、御同病のため御医師武田長春院へ御転薬された

一、同月十七日、右同様の状況であったが、御変症で御重篤の様子のため、出羽守様が御見舞いをされ、御医師村田長安老へ御依頼して御転薬された

一、同日、右同様の状況であったが、夜の亥下刻（午後10時頃）に御逝去された。御法名は

一、同月十八日、右同様のことについて御用番松平右近将監武元殿へ御届けとして、御留守居津田九右衛門盛恕が左記の通り書付を持参して差し出した
