一 同十三辰年三月小番頭被 仰付候事
一 同年十月江戸為勤番罷登翌巳五月
御下向御供ニ而罷下候
但陣中御小袖壱ツ御上下壱具労銀拾
枚被下置候事
一 同十六未年六月御小姓頭被 仰付候事
一 同年八月最寄勝手ニ付大原六太夫屋敷と
相対替仕度旨被伺願候事
一 宝永元申年三月江戸為勤番罷登同
九月罷下候
但陣中御馬 御献上御用相勤此後壱
度相勤候事
一 同二酉年五月壱ツ者月星有之壱ツ者叶と言
文字有之銭弐銅差上候事
一 同年六月千葉和助と相改度旨被伺願候
但本苗千葉ニ御座候よし申上候處千葉者御
家中ニ茂無之他所ニも無之事ニ候旨旁早
之名乗候而能候半右之願申出候様ニと
御懇之御意奉蒙候ニ付奉願候よし
現代語訳
件
一 同十三辰年三月 小番頭を仰せ付けられた件
一 同年十月 江戸勤番のため上り、翌巳五月に
御下向の御供として帰国した
但し陣中御小袖一つ、御上下一具、労銀十
枚を下し置かれた件
一 同十六未年六月 御小姓頭を仰せ付けられた件
一 同年八月 最寄りの都合により大原六太夫屋敷と
相対替えをしたい旨を伺い願い出た件
一 宝永元申年三月 江戸勤番のため上り、同
九月に帰国した
但し陣中御馬 御献上の御用を相勤め、この後一
度相勤めた件
一 同二酉年五月 一つは月星があり、一つは叶という
文字がある銭二銅を差し上げた件
一 同年六月 千葉和助と改めたい旨を伺い願い出た件
但し本苗は千葉にございますと申し上げたところ、千葉は御
家中にもなく他所にもない事でございますが、とにかく早
くの名乗りでありますから良いでしょう、右の願いを申し出るようにとの
御懇意の御意を蒙りましたので願い出ますとのこと

Note: Was granted one kosode robe for military service, one set of formal court dress, and ten pieces of reward silver
1. Same 16th year of Hitsuji (未), 6th month: Was appointed to the position of okosho-gashira (chief page)
1. Same year, 8th month: Petitioned to exchange residences with Ohara Rokudayu's mansion for convenience of location
1. Hoei 1st year of Saru (申), 3rd month: Went up for guard duty in Edo, returned home in the 9th month of the same year
Note: Served in military duties related to horse presentation and献上, and served once more thereafter
1. Same 2nd year of Tori (酉), 5th month: Presented two copper coins, one bearing moon and star designs, one bearing the character "kano" (叶)
1. Same year, 6th month: Petitioned to change name to Chiba Wasuke
Note: When he stated that his original family name was Chiba, he was told that while there was no Chiba family among the retainers nor elsewhere, since it was an early name adoption, it would be acceptable, and he was kindly advised to submit this petition, which he accordingly did
