以上の通り、名前替わりあるいは苗字替わりの者については、これまでの苗字または名前の箇所に点（削除印）を付し、その脇に新しい名前を記入すること。
一、たとえ立替（家督相続等による交代）に際して苗字とともに改めた者についても、これまでの記載箇所に点を付し、その脇に現在の姓名を記入すること。
一、小頭が病死し、末席の者であっても小頭に昇格した場合は、病死した小頭の名前に点を付し、そのまま昇格した者の姓名の上に「小頭に相成」と記入すること。
一、その他の場合は以上に準じて処理すること。なお、席次が不明な場合は差し支えないので、札紙の張替えや御札等の改めは行わないこと。
一、諸下代のうちで入替等があった場合は、誰それの下代の箇所に誰それの下代に相成と記入すること。なお、図示の通りに行うこと。

一、六月十九日、御持筒組の者の名元（名簿登録）については、これまで御用人より申し送りがあれば御目付方において吟味の上、通達してきた。しかるに当年より諸組および御駕籠（御駕）の者に至るまで名元の吟味を御目付が引き受けるよう仰せ付けられ、すなわち御帳（名簿）相九冊が御用人より回付された。これにより、以下の通り姓名書付を提出するよう、それぞれ月番・宮北長左衛門の屋敷へ招集し、書付をもって申し達した。またその際、組の者を召し抱える（雇用する）際の心得等についても一紙に書き載せ、来る二十六日までに提出するよう申し達しておいたところ、順次提出があったため、御帳の名元に相違がある分については加筆訂正を行い、大宮彦右衛門へ通達した。
一、今後、組の者を召し抱える際には、御定めの年数が満了していない場合はもちろんのこと、満了した場合であっても、すべて問い合わせの上で召し抱えるべき旨である。
一、組の者の名元書（名簿）は、これまでは御用人まで提出されていたが、今後は御目付まで提出すること。なお、記載の方法については、これまで御用人へ提出していた通りで差し支えない旨である。
以上の趣旨を以下の面々へ通達した。

As stated above, for those who have changed their given name or surname, a mark (deletion dot) shall be placed over the previous surname or given name in the register, and the new name shall be written alongside it.
Item: Even in cases where both the surname and given name have been changed due to succession (tatekae), a mark shall be placed over the previous entry and the current full name shall be written alongside it.
Item: If a squad leader (kogashira) dies of illness and even the most junior member is promoted to squad leader, a mark shall be placed over the deceased squad leader's name, and the notation "promoted to squad leader" shall be written above the name of the person who has been promoted as-is.
Item: All other cases shall be handled in accordance with the above. Note that when the order of seating/rank is unclear, there is no need to replace the label papers (fuda-gami) or revise the official placards (o-fuda), as this presents no problem.
Item: When there are replacements among the various deputies (shitadai), it shall be written that so-and-so has become the deputy in place of so-and-so. The format shall follow the diagram provided.

Item: On the 19th day of the 6th month, regarding the name registration (naamoto) of members of the O-mochizutsu-gumi (Musket Bearer Unit), until now, when notification came from the Go-yōnin (Senior Administrators), the matter would be investigated by the Go-metsuke (Inspectors) and then communicated. However, from this year onward, it has been ordered that the Go-metsuke shall take charge of investigating the name registrations for all units (shogumi) as well as the O-kago (palanquin bearer) personnel. Accordingly, nine volumes of the official registers (o-chō) were forwarded from the Go-yōnin. In light of this, each party was summoned to the residence of the monthly duty officer (tsukiban) Miyakita Chōzaemon, and they were notified by written document to submit their name registers (seimei kakitsuke) in the manner prescribed below. At that time, instructions regarding the procedures for hiring (meshikakaeru) unit members were also set down in a single document, and the parties were instructed to submit these by the 26th of the coming month. As submissions arrived in succession, corrections and additions were made to those entries in the official register that were found to be in error, and the updated information was communicated to Ōmiya Hikoemon.
Item: Henceforth, when hiring members for a unit, not only in cases where the prescribed term of service has not yet been completed, but even in cases where it has been completed, all hirings shall be carried out only after proper inquiry has been made.
Item: The name registers (naamoto-gaki) of unit members had previously been submitted to the Go-yōnin, but henceforth they shall be submitted to the Go-metsuke. The manner of recording shall remain the same as previously submitted to the Go-yōnin.
