事
一、同十六亥年四月、御下向の御供を仕る筈であったが、病気のため御先番として退城した事
一、同年九月十三日、三十二歳にて病死。法名は樹闇院応誉随現居士、葬所は同上
久左衛門重倚（初名は初之助、又左内。享保八卯年十二月朔日誕生）
母 荒川氏の娘
妻 石山郷右衛門安禎の娘（享保十四酉年九月十四日誕生、文化五辰年正月七日八十歳にて病死、法名は重頼院随誉貞順大姉、葬所は同上）
一、元文四未年十月、親の願いの通り初めての御目見を仰せ付けられた事
一、同月、親の願いの通り御番代を仰せ付けられ、沼沢権兵衛組にて御城での御番を相勤めた事
