頼寧君御代
一 文政七申十月十三日新規被 召出御宛行
拾人扶持被下之儒学専ニ相心得医業も
可相勤候
一 天保六未二月十三日伜郁之丞
思召を以 学被 仰付依之往々《場所：江戸》表
佐藤捨蔵江随身仕為致修行可申候
一 同七申四月廿三日以後御医師当番打込勤并
祥橘様御住居勤之処も外並之通泊も

中村中書
頼寧君御代
一、文政七年申年十月十三日、新規に召し出され、十人扶持を下され、儒学を専門に心得、医業も勤めるべきこととされた。
一、天保六年未年二月十三日、息子郁之丞に思し召しをもって学問を仰せ付けられ、これにより将来江戸表において佐藤捨蔵に随身して修行を行うべきこととされた。
一、同七年申年四月二十三日以後、御医師当番打込勤務並びに祥橘様御住居勤務の際も、他と同様に泊まりも勤めるよう仰せ出された。
一、同十年亥年十一月六日、不敬不束な事柄があり、逼塞を仰せ付けられた。
一、同月二十五日、逼塞を御免となった。
一、弘化二年巳年十二月十日、病気の上に老年につき、願いの通り隠居を仰せ付けられ、息子郁之丞に家督を相違なく下された。

Nakamura Chūsho
During the reign of Lord Yoriyasu:
1. On the 13th day of the 10th month of Bunsei 7 (1824), he was newly summoned to service and granted a stipend of ten person-portions, with the understanding that he would specialize in Confucian studies and also practice medicine.
1. On the 13th day of the 2nd month of Tenpō 6 (1835), his son Ikunosuke was appointed to studies by special consideration, and it was decided that in the future he should accompany Satō Sutezō in Edo to pursue his training.
1. From the 23rd day of the 4th month of the same 7th year (1836) onward, he was ordered to perform overnight duties like others when serving as court physician on duty and when serving at Lord Shōkitsu's residence.
1. On the 6th day of the 11th month of the same 10th year (1839), due to disrespectful and improper conduct, he was ordered into house confinement.
1. On the 25th day of the same month, he was pardoned from house confinement.
1. On the 10th day of the 12th month of Kōka 2 (1845), due to illness and advanced age, he was granted retirement as requested, and the family headship was transferred without issue to his son Ikunosuke.
