右の者は代々浄土宗であり、当寺の檀家に間違いございません。もし御制禁のキリシタン宗門やその同類、すべての御法度の宗門に属する人がおりましたら、私（住職）はどこまでも出向いて必ず申し上げます。後日のため一札を差し上げます。以上の通りです。

The aforementioned person has been of the Jōdo sect for generations and is without doubt a parishioner of our temple. Should there be any person who belongs to the prohibited Christian sect, its associates, or any other forbidden religious sects, I (the priest) will go anywhere and definitely report this matter. I hereby provide this written statement for future reference.
