て物まへ大晦際（おゝつごもりぎわ）にいたりて買懸（かいかゝ）りと同しことに

生活とともに、とりわけ医者の心労したことを忘れて、大晦日間際になってから買い掛けと同じことのように思えて、その時になって困窮するのである。およそ医者の腰が重く、急な時に間に合わないのは、内々謝礼が薄いことが原因である。常に得意の医者には季節の物の贈り物を、老医には寒暑の見舞いも絶えず送り、病気になった時はひたすら信仰し、その治療が終わったら早く謝礼を厚くするなら、自然と腰の重い医者も軽やかになるものである。腰が重いのには二種類ある。一つには病家が常に疎遠であるか、または不人気の医者はたまたま呼びに来ることがただ一軒ばかりに衣服を着替え、僕の縄ないを止めさせるのも面倒なので、多くは「承知いたしました」と返事して腰を上げないものである。四十を過ぎた医者は特に、寒い夜に寝て温まっているところを夜中に叩き起こされて、舌打ちを二三百回して、家内の者たちを起こし「火を起こしておけ、湯を沸かしておけ」と不時の準備費用があっても、病家はまったく気がつかないことである。けれども謝礼の厚い病家へは雪を踏んで走って行くのは、皆頂に物欲しがる病が医者にあるからである。

Generally, when doctors are slow to respond and cannot arrive promptly in urgent situations, it is due to meager compensation given privately. If one regularly sends seasonal gifts to favored doctors, never fails to send greetings during hot and cold seasons to elderly doctors, faithfully trusts them when illness arises, and provides generous compensation promptly after treatment ends, even slow-moving doctors will naturally become more responsive. There are two types of sluggishness. First, when the patient's family is habitually indifferent or when an unpopular doctor is occasionally called to just one house, changing clothes and stopping the servant's rope-making is troublesome, so they often just reply "I understand" and don't move. Doctors over forty, especially when awakened by knocking in the middle of a cold night after warming up in bed, click their tongues two or three hundred times and tell household members to "light the fire, boil water," incurring unexpected preparation costs that patients never notice. However, they will run through snow to wealthy patients' homes because doctors all suffer from the ailment of greed.
