現代語訳
等は皆山帰来を主とする。この故に私は奇良湯一将を用いる。いずれも比類なき勇将であるが故に、私はこれを以て四天王とし、他の薬を以てこれが助けとする。
およそ梅毒を攻め滅ぼすには、この四天王を用いて事足りる。他の方剤は機に臨み変に応じて、これが加勢とすべきである。私はまた梅毒対治の実験がある。延寿丸、治瘡丸、梅効散の三大将を用いて合戦に及ぶ時は、梅毒賊徒の五臓六腑・骨髄に攻め入った者も散乱して表に発し、傷のある所は四、五日は痛みを発し、腫れを生じ、膿汁が出ることがある。これは梅毒賊徒を駆除して肌肉を上げるが故である。梅賊が逃げ去るに従い痛みは去り、膿水は尽き、傷は癒えて太平となる。
また国によっては、内にある梅毒が延寿丸或いは治瘡丸或いは梅効散に攻め立てられ、表に出て熱を発し、斑を発し、四、五日に逃げ去ることがある。これを以て見れば、発表の甚だしき温薬である。汝等のような者は水銀を以て一向に寒薬と思うこそおかしい。薬は製法による者である。
米と麹と水とを製法によって甘酒となり、酢となり、酒となり、焼酎となる。この四つは冷水を以て造るけれども、熟する時は熱物となる。汝等は冷水と米とを以て造る酒も寒薬とするか。延寿丸、治瘡丸、梅効散、この三将を私は温熱の発表剤とする。この三将の武勇はよく疾病を破り、梅毒賊徒を駆除することは神の如しである。
私が常に世医のなすところを見るに、例えば下疳、楊梅瘡、或いは骨痛、或いは筋痛、或いは頭痛、或いは四肢・胸・背・腰・腹の傷、或いは咽・鼻等の痛みに諸々の劇薬を用いて大効を現し、外に現れるところの傷、或いは腫れ、或いは痛みさえ治すれば、病根は抜けたような様に思い、未だ梅毒が尽きないのに大功をなした薬将をやめ、薬毒を抜くと称して山帰来剤を用いるのは大いなる誤りである。

All of these primarily use smilax (sankirai). For this reason, I employ the single general Kiryōtō. Since all of these are peerless brave generals, I make them my Four Heavenly Kings and use other medicines as their support.
Generally speaking, to attack and destroy syphilis poison, using these Four Heavenly Kings is sufficient. Other prescriptions should be used as reinforcements when facing critical situations and responding to changes. I also have practical experience in treating syphilis. When the three great generals—Enjūgan, Jisōgan, and Baikōsan—engage in battle, even syphilis bandits that have invaded the five organs, six viscera, and bone marrow scatter and emerge to the surface. Areas with wounds may develop pain for four to five days, produce swelling, and discharge pus. This is because the syphilis bandits are expelled and the flesh rises. As the syphilis bandits flee, the pain subsides, the pus ceases, the wounds heal, and peace is restored.
Also, depending on the constitution, internal syphilis poison, when attacked by Enjūgan, Jisōgan, or Baikōsan, may emerge to the surface, causing fever and rashes, then flee within four to five days. From this we can see that these are warm medicines with strong surface-releasing properties. It is absurd that people like you think of mercury as purely a cold medicine. Medicine depends on its preparation method.
Rice, koji, and water, depending on the preparation method, become sweet sake, vinegar, wine, or distilled spirits. Although these four are made with cold water, when they mature they become hot substances. Would you also classify wine made from cold water and rice as a cold medicine? I classify these three generals—Enjūgan, Jisōgan, and Baikōsan—as warm surface-releasing agents. The martial prowess of these three generals in breaking diseases and expelling syphilis bandits is godlike.
When I observe what contemporary physicians do, for example, with chancres, syphilis sores, bone pain, muscle pain, headaches, wounds on the limbs, chest, back, waist, and abdomen, or pain in the throat, nose, etc., they use various strong medicines to achieve great effects. Once they cure the external manifestations—wounds, swelling, or pain—they think the disease root has been extracted. Even though the syphilis poison has not been completely eliminated, they stop using the medicinal generals that achieved great success and instead use smilax preparations under the pretext of eliminating drug toxicity. This is a grave error.
