透公様高遠御入国之節先規ゟ居
住之士依御招光武兄弟三人被召出三百
石ツヽ被下作左衛門義ハ鎗組相勤罷
在候事
一同十二申年松本之城主小笠原右近太夫貞慶
乱入之時罷出防戦いたし候事
一同十三酉年八月
権現様真田安房守昌幸か上田之城御攻撃之
時鉄炮大将ニ而出陣いたし上田之城大手之門際迄
押詰候時城門を開上田勢突出仕候ニ付味方敗
軍ニ及引取候処加野川と申所迄崩候節敵軍慕
来追討致候ニ付 透公様御馬を被為《見せ消ち：進｜返》敵
軍を被追払候時作左衛門討死いたし候法名葬所不詳
此法名を位列香蓮寺と申寺ニ過去帳有之
其写之由に候へとも誰法名と申事も年月も無之
難相分併後年為参考出之

現代語訳
透公様（保科正之公）が高遠にご入国の際、以前よりその地に居住していた士が召し出しの命に応じ、光武兄弟三人が召し出され、それぞれ三百石を拝領した。作左衛門については鎗組を勤め仕えた。
一、同十二年（天正十二年・申年）、松本城主・小笠原右近大夫貞慶が乱入した際に、出撃して防戦に当たった。
一、同十三年（天正十三年・酉年）八月、
権現様（徳川家康公）が真田安房守昌幸の上田城をご攻撃の際、鉄炮大将として出陣し、上田城の大手門際まで攻め詰めたところ、城門を開いて上田勢が突出してきたため、味方が敗走することとなった。引き取る途中、加野川と申す所まで崩れた節に、敵軍が追いかけてきた。その時、透公様が御馬をお返しになり敵軍を追い払おうとされた際に、作左衛門は討死した。法名・葬所は不詳。
この法名については、位列香蓮寺という寺の過去帳に記載があるとされる。その写しによるものとのことだが、誰の法名であるか、また年月も記されておらず、判別しがたい。しかし後年の参考のために掲載する。

When Lord Tōkō (Hoshina Masayuki) entered Takatō, those samurai who had previously resided in the area were summoned, and Mitsutake and his two brothers were called forth together, each receiving three hundred koku. Sakuzaemon served in the spear corps (yarigumi).
One entry: In the same twelfth year [Tenshō 12, Year of the Monkey, 1584], when Ogasawara Ukon-no-tayū Sadayoshi, lord of Matsumoto Castle, made an incursion, [Sakuzaemon] went out and engaged in defensive combat.
One entry: Eighth month of the same thirteenth year [Tenshō 13, Year of the Rooster, 1585]:
When Gongen-sama (Lord Tokugawa Ieyasu) attacked the castle of Sanada Awa-no-kami Masayuki at Ueda, [Sakuzaemon] sortied as commander of the arquebusiers (teppō-taishō). When the forces pressed forward all the way to the main gate (ōtemon) of Ueda Castle, the castle gate was opened and the Ueda forces burst out, causing our side to suffer defeat and retreat. As [our forces] fell back and collapsed as far as the place called Kanogawa, the enemy army pressed in pursuit. At that time, Lord Tōkō turned his horse around to drive off the enemy forces, and during this action, Sakuzaemon was killed in battle. His posthumous Buddhist name (hōmyō) and burial place are unknown.
Regarding this posthumous name: it is said that there is a record in the death register (kakochō) of a temple called Iretsu Kōrenji. Although it is a copy from that register, neither whose posthumous name it is nor the date is recorded, making it difficult to determine. Nevertheless, it is included here for reference in later years.
