右乱心領知被召上御息又四郎殿へ五千石次男造酒之丞へ千石被下之候
右乱心之次第佐渡守殿家来朱雀平助と申者不届有之由にて
逼塞申付被差置候処此節佐渡守殿祖父因幡守殿百年忌被相
当候ニ付家老藤田八郎右衛門と申者右平助御免之儀詫申候処佐渡守殿
以之外立腹被致平助同意之不届者之由ニて左之通切腹ニ申付候

家老大石内蔵助を頭として吉良上野介殿本所屋敷へ仕込両門より
討入寝所へ乱入主人之仇上野介殿を討一所ニ引取内匠頭殿菩提所
芝泉岳寺へ集り居右之趣意以書付御目付仙石伯耆守殿へ申達候
但上野介殿首間十次郎と申もの取之と云
吉良佐兵衛殿家来之内十六人死其内十一人は刀切込血付有之と云
右四十六人大名衆御預ケ 十七人細川越中守殿
拾人ツヽ毛利甲斐守殿松平隠岐守殿 水野監物殿 右之通御預
にて翌未年二月切腹被 仰付候也

現代語訳
右の乱心により領知を召し上げられ、御息子又四郎殿へ五千石、次男造酒之丞へ千石を下された。
右の乱心の次第は、佐渡守殿の家来朱雀平助という者に不届きがあったということで逼塞を申し付け差し置かれていたところ、この節佐渡守殿の祖父因幡守殿の百年忌に相当するにつき、家老藤田八郎右衛門という者が右の平助の御免の儀を詫び申したところ、佐渡守殿は以ての外立腹され、平助同意の不届き者ということで左の通り切腹に申し付けた。

右の通り申し付けられた儀を親類衆も存じておらず、その段を相聞いたにつき親類衆が見舞いを申されたところ、様子が乱心の体に相見えたにつき言上があった由。右の者共が切腹を申し付けられた砌まで、家来共まで曽て乱心の体と存ぜず心付きもなかった由である。

但し上野介殿の首は間十次郎という者が取ったという。
吉良佐兵衛殿の家来の内十六人死に、その内十一人は刀の切り込みに血付きがあったという。

十人ずつ毛利甲斐守殿、松平隠岐守殿、水野監物殿 右の通り御預けにて翌未年二月切腹を仰せ付けられた。

右の者共が書付を持参し相達した由。

Due to his mental illness, his domain was confiscated, and his son Matashirō-dono was granted 5,000 koku, while his second son Mikawa-no-jō was granted 1,000 koku.
The circumstances of this mental illness were as follows: A retainer of Lord Sado-no-kami named Suzaku Heisuke had committed some impropriety and was placed under house confinement. At this time, as it corresponded to the centennial memorial of Lord Sado-no-kami's grandfather Lord Inaba-no-kami, a chief retainer named Fujita Hachirōemon petitioned for Heisuke's pardon. Lord Sado-no-kami became extremely angry and, considering him an accomplice of the improper Heisuke, ordered the following men to commit seppuku:

The relatives were unaware that such orders had been given, and when they heard about it and came to visit, the situation appeared to show signs of mental illness, which was reported. Until the moment these men were ordered to commit seppuku, even the retainers had never noticed any signs of mental illness.

○Same year, night of December 15th: Forty-six retainers of the late Lord Asano Takumi-no-kami <<However, of forty-seven men, one was a foot soldier who requested to withdraw from the scene>>
Led by chief retainer Ōishi Kuranosuke, they infiltrated the Honjo mansion of Lord Kira Kōzuke-no-suke, attacked through both gates, burst into his sleeping quarters, killed Kōzuke-no-suke in revenge for their master, withdrew together, and gathered at Sengakuji Temple in Shiba, the family temple of Lord Takumi-no-kami. They submitted a written statement explaining their intentions to the metsuke Lord Sengoku Hōki-no-kami.
However, it is said that Kōzuke-no-suke's head was taken by a man named Ma Jūjirō.
Among Lord Kira Sahyōe's retainers, sixteen died, of whom eleven showed blood on their sword cuts.
The above forty-six men were placed in the custody of daimyo: Seventeen with Lord Hosokawa Etchū-no-kami; Ten each with Lord Mōri Kai-no-kami, Lord Matsudaira Oki-no-kami, and Lord Mizuno Kenmotsu. Under this custody arrangement, they were ordered to commit seppuku in February of the following year of the Sheep.
Of the forty-seven men who went to the senior metsuke, two men <<District administrators: Yoshida Chūzaemon, age 63; Tomimori Sukeemon, age 32>>
These men reportedly brought and submitted written documents.
