**[Right folio]**
There is no account in the writings of the ancients of purging with *shien* (purple pills). If one mistakenly administers a purgative to a patient with a deficiency condition (*kyoshō*), it will kill them. Furthermore, for smallpox, all manner of so-called "miraculous remedies" and "extraordinary prescriptions" sold as patent medicines must never be used. The ancient saying, "Not medicating the ill is the achievement of a middling physician," refers precisely to cases such as these.
One note: When a child develops convulsions during the initial fever stage, laypeople panic and apply moxibustion, while physicians administer *shien* under the guise of a "restorative." Both are errors. They will invariably cause great harm afterward. Simply put, once smallpox erupts, the convulsions will resolve on their own without treatment. If one cannot bear to merely sit and watch, bear bile (*kumanoi*) may be administered.
**[Left folio]**
Also, bathing when one has even a slight fever causes great harm. Even laypeople know well that bathing, moxibustion, and *shien* are harmful when there is a fever from a cold. Yet there are even physicians who do not understand the reason why these are most harmful during the initial fever of smallpox. This is entirely because they are unfamiliar with texts on smallpox.
One note: In recent years, Dutch studies (*Rangaku*) have flourished, and there are those who frequently prescribe *kinakina* (quinine/Cinchona bark). Among my father's disciples, there is one who has a deep passion for Dutch studies and traveled to Nagasaki last year to study under Dutch physicians. According to his view, during the initial fever of smallpox and the crusting stage (*kase*), purgatives should be used…
