私儀不調法者御座候所数拾年奉蒙 御厚恩段々
結構被 召仕被下置其上追々御加米等頂戴仕
重量冥加至極難有仕合奉存候然ル上者何分出
情仕及公力候然者御奉公成上度公願罷在候処風与
病気ニ付以 御威光種々療養仕候得共一躰難渋之
病症ニ而存命之程難斗奉存候万一落命仕候者此上之
以 御慈悲倅伊平治儀及渇命不申候様奉願上候
右之趣
御支配中様江宜様被仰上可被下候以上

現代語訳
私は不調法者でございますが、数十年の間、厚いご恩恵をいただき、段々と結構にお召し使いいただき、その上、追々とご加米なども頂戴いたし、重ね重ね冥加至極でありがたい仕合わせと存じております。そうである以上は、何分にもお役に立ち、お力になりたく存じます。そして、ご奉公を成し上げたく願っておりましたところ、風邪と病気のため、ご威光をもって種々療養いたしましたが、一体に難しい病症で、生存の程度も測り難く存じております。万一命を落とすことがございましたら、この上のご慈悲をもって、倅の伊平治が渇命（困窮）することのないよう、お願い申し上げます。
右の趣旨を、ご支配中の皆様へよろしく申し上げていただけますよう、お願いいたします。以上

英語訳
I am an incompetent person, but for several decades I have received your generous benevolence and have been employed most excellently by you. Furthermore, I have gradually received additional rice stipends and other benefits, for which I am extremely grateful and consider myself most fortunate. Given this, I wish to be of service and assistance to you in any way possible. I have been hoping to fulfill my duties faithfully when I fell ill with a cold and sickness. Despite receiving various medical treatments through your gracious influence, this appears to be a serious illness, and I fear my chances of survival are uncertain. Should I pass away, I humbly request that through your continued mercy, my son Iheiji will not fall into destitution.
I respectfully ask that you convey the above matter favorably to all those in authority.
