載候所武右衛門勤中実父太左エ門
旧功御立被下候義も相見勿論訳

現代語訳
記載されていないところ、武右衛門が勤務中、実父太左衛門の旧功をお立てくださった義も相見え、もちろん訳のある者につき、大左衛門の由来を記し置く事。
一、寛永十六年卯年二月、江戸において並の御擬作を下され、御徒に召し出された事。
一、同二十年未年八月、御入部の節に御供仕り罷り下ったところ、久々江戸に詰めていたにつき、正保元年申年まで休息仕り罷り在った事。
一、正保元年申年八月、御譜代の者、その上例々能き者の筋に候間、岩彦様へ遣わされ候。内々勝手次第に支度仕り、江戸表へ罷り登るべく候。末々は御取り立て下さるべき由の御意の由仰せ出され候事。
一、同年十月、江戸表へ罷り登った事。
一、慶安元年子年四月、養父善兵衛の願いの通り養子に仰せ付けられた事。
一、同四年卯年四月、岩彦様の御懐守に仰せ付けられた事。

[Continuing from] what was not recorded, while Takemon was in service, his biological father Tazaemon's past meritorious service was also recognized, and of course being a person with good reason, the origins of Tazaemon are recorded here.
1. In Kan'ei 16, year of the rabbit, 2nd month, in Edo he was granted the regular stipend and summoned to serve as a foot soldier (ashigaru).
1. In the same [Kan'ei] 20, year of the sheep, 8th month, he served as attendant during the lord's entry into the domain and went down [to Aizu], but since he had been stationed in Edo for a long time, he remained at rest until Shōhō 1, year of the monkey.
1. In Shōhō 1, year of the monkey, 8th month, being a hereditary retainer and moreover from a lineage of consistently capable men, he was sent to serve Iwahiko-sama. [He was told] to make preparations privately as convenient and proceed to Edo, and that eventually he would be promoted, according to his lordship's intention.
