無役組に仰せ付けられた事。
一、同四年午年二月、組付に仰せ付けられたく願い申し上げたところ、その通りに罷り有るようにと仰せ付けられた事。
一、同五年未年三月、再願の上、外様組付に仰せ付けられ、上田八郎右衛門組へ入り、御城御番を相勤めた事。
一、享保元年申年九月十一日、五十五歳にて病死。法名は深源心院唯観日成。葬所は同上。

一、宝永元年申年二月、実家に罷り有る節、並みの御擬作八両四人扶持を下され置かれ、御小性に召し出された事。
一、正徳二年辰年七月、願いの通り養子に仰せ付けられ、只今まで通り御小性を相勤めるよう
英語訳
He was assigned to the no-duty group (muyaku-gumi).
- In the 4th year (Shōtoku 4, 1714), 2nd month, when he petitioned to be appointed to a group assignment (kumitsuki), he was told to remain as he was.
- In the 5th year (Shōtoku 5, 1715), 3rd month, after petitioning again, he was appointed to an external group assignment (tozama-kumitsuki), entered Ueda Hachirōemon's group, and served castle guard duty.
- In Kyōhō 1 (1716), 9th month, 11th day, he died of illness at age 55. His Buddhist name was Shingen-shin-in Yuikan Nissei. Burial place: same as above.

Actually the second son of Koizumi Tsuneemon [a certain person]
- In Hōei 1 (1704), 2nd month, while staying at his birth family, he was granted the standard stipend of 8 ryō and support for 4 people, and was called to serve as page (koshō).
- In Shōtoku 2 (1712), 7th month, as petitioned, he was appointed as adopted son and was to continue serving as page as before.
