右の通り差し出されたところ、同日夕方八時に相模守殿より御呼び出しがあり、御留守居・大谷一右衛門《割書：良次》が罷り出たところ、小五郎様御改名の儀、御伺いの通り仰せ出される旨を仰せ聞かされた。右の御礼のために、宗矩様は御用番・酒井雅樂頭《割書：忠知》殿ならびに相模守殿へ（直接）相勤め、残る御老中へは御使者として御留守居を差し出された。
一、同日、小五郎様が於義丸と御改名なされたことにつき、宗矩様より御使いとして御中老・明石将監《割書：紀房》をもって、右の御名目録ならびに鮮鯛一折を進呈された。
御口上
御名の儀、御老中へ御伺いなされたところ、御伺いの通り仰せ出されたため、折紙をお認めになり進呈申し上げます。御先祖様の御名でございますため、めでたく御祝いとして御肴一種を添えて進呈申し上げます。

The above document having been submitted, on the same day at the eighth hour of the evening [approximately 10 p.m.], a summons was received from Lord Sagami-no-Kami, and the resident keeper (orusui) Ōtani Ichiemon 《marginal note: Yoshitsugu》 went to attend. He was informed that the name change of Lord Kogorō had been approved as requested. In order to express gratitude for this, Lord Munenori personally called upon the duty Senior Councilor (goyōban), Lord Sakai Uta-no-Kami 《marginal note: Tadatomo》, as well as Lord Sagami-no-Kami, and dispatched the resident keeper as a messenger to the remaining Senior Councilors.
1. On the same day, as Lord Kogorō had been renamed Yoshimaru, Lord Munenori sent his middle elder (ochūrō), Akashi Shōgen 《marginal note: Norihusa》, as a messenger, delivering the name register (omyō mokuroku) together with one box of fresh sea bream.
Oral message (okōjō):
Regarding the matter of his name: having submitted an inquiry to the Senior Councilors, and having received approval as requested, a formal document (origami) has been prepared and is hereby presented. As this is a name that belonged to an ancestor, this is presented as a celebratory offering, with one item of accompanying food (osakana) added.
