【右丁】
是 神（かみ）の教（をしへ）に出（いで）て。昔（むかし）より
伝（つた）へ来（きた）れる方（みち）なり。爰（こゝ）に吾（わが）
師（し）。静観（じやうくわん）老人 諸（もろ〳〵）の書（ふみ）見る
度（たび）に。此疾（このしつ）をまぬかるゝ法
あれば。必（かなら）す書留（かきと）め且（かつ）家々（いへ〳〵）
に秘（ひ）せし術（じゆつ）をも。千（ち）たび乞（こ）ひ
百（もゝ）たび求（もと）め。伝（つた）え。請（うけ）て書（かき）つ
づりし一小 冊（さつ）あり《割書：僕（やつかれ）》世の人の
子（こ）を思ふ闇（やみ）を照（て）らし。心の
とかなく。やすらかにあらん事
を思ふが故に。写（うつ）し取（とり）て書林

にあたへぬ。見ん人 浅（あさ）はかなる
事に思はで。此おしへの如く
せば。大己貴（をゝあなむち）。少彦名（すくなひこな）の。神（かみ）
慮（こゝろ）にもかなひて。児女（じちよ）を
して齢（よはひ）長（なかく）く【衍】久しく。目
出たかるべし。豈（あに）疑（うたが）ふべけん

現代語訳
【右丁】
これは神の教えに基づいて、昔から伝えられてきた方法である。ここに我が師である静観老人が、様々な書物を見るたびに、この疾患を免れる方法があれば必ず書き留め、かつ各家で秘蔵されている術をも、千度も請い百度も求めて、伝え受けて書き綴った一冊の小册子がある《私が》。世の人が子を思う闇路を照らし、心の苦悩なく、安らかであることを願うがゆえに、写し取って書肆に与えるものである。

これを見る人は、浅はかなことと思わずに、この教えの通りにすれば、大己貴命・少彦名命の神意にもかない、児女を長寿にして、めでたいことであろう。どうして疑うことがあろうか。

These are methods that emerge from the teachings of the gods and have been transmitted since ancient times. Here, my teacher, the elderly Jōkan, whenever he saw various books, would always record any methods for avoiding this disease, and also sought thousands of times and requested hundreds of times the secret arts hidden in various households, transmitting and receiving them to compile into one small booklet.《I》, in order to illuminate the darkness of parents' worries for their children throughout the world and to wish that their hearts be free from anxiety and peaceful, have copied this and given it to the bookseller.

Those who read this should not think of it as shallow matters, but if they follow these teachings, they will accord with the divine will of Ōanamuchi and Sukunahikona, and their children will live long lives and be blessed. How could one doubt this?
