第二十一章 初起平症治法

である。すなわち発生・醸熟・分利がこれである。およそこの疫病においては、初めの七日を発生と定めて、病が増長する時期であり、中の七日を醸熟と定めて、病毒が化熟する時期であり、後の七日を分利と定めて、病毒が謝出する時期である。しかし元運自然の良能が健全でなくて、熟成させることができないか、または仮に熟したとしても、元運がその労に勝たずして、分利することができないか、いずれもその終わりは成敗の二途に出るのみである。なお痘瘡に序熱・見点・出斉・灌膿・結痂を立てて論ずるのと同じである。これは医見の標準を立てて示すものであって、治疾察病の用に設ける。すべて治療の大旨は運化の機転を扶けて、自然分利の功を遂げ成させることにある。余がこれを熟思するに、熱病に限らずすべて皆ある。そもそも天地一切の事物にも本末終始があって、盛んであれば衰え、生ずれば死するの理があるのと同じである。高明の君子は我が蘭説を待たずとも早くすでに了解するであろうが、今ここに載せるのは、近来ヘッケルの書を見て、喟嘆の余り贅言を記して同志に示すのみである。

That is to say, these are the stages of initial development, maturation, and resolution. Generally, in this epidemic disease, the first seven days are designated as the developmental stage when the disease increases and grows; the middle seven days are designated as the maturation stage when the pathogenic toxin undergoes transformation and ripening; and the final seven days are designated as the resolution stage when the pathogenic toxin is expelled from the body. However, if the natural vital function of primordial movement is not robust and cannot achieve maturation, or even if it does mature, if the primordial movement cannot withstand that labor and cannot achieve resolution, then ultimately the outcome can only lead to one of two paths: success or failure. This is similar to how smallpox is analyzed by establishing the stages of initial fever, spot appearance, full eruption, pustulation, and scab formation. This establishes a standard for medical observation and is set up for use in treating diseases and examining illness. The main principle of all treatment lies in supporting the mechanism of transformation and circulation to achieve and accomplish the natural function of resolution. When I reflect deeply on this, it exists not only in febrile diseases but in all conditions. Indeed, just as all things in heaven and earth have beginning and end, start and finish, and follow the principle that what flourishes must decline and what is born must die. Enlightened gentlemen would understand this early on without waiting for our Dutch explanations, but what I record here is merely additional words written for like-minded colleagues after reading Heckel's recent book with deep sighs of admiration.
