現代語訳
速やかに下痢を止めるべきである。甚だしい場合は一、二日で死に至る。阿片でなければ効果がない。私はこの症状に初めから軽量の阿片を数回に分けて用い、第二揮発飲を併用して効果を得ることが多い。およそ下痢があるといえども、熱が激しくて焮腫を伴う場合には、阿片・附子共に用いることを許さない。
○碩按：附子は味が辛く、かつ神経を刺激して麻痺を回復させ、痙攣を鎮め汗を発する。しかしその性質は激烈で、量が多ければ人を麻痺廃絶させ、血脈を傷つけ火気を旺盛にする。軽量を連続して進めるのがよい。
第二十三章 重症治法
病勢がかなり進行し、脈数にして弱く、昼夜純粋に熱があり、妄語・撮空があり、筋肉が痙攣し震え動き、下痢・嘔逆がある場合は、この湯を用いるのがよい。
第二揮発飲（ウィリス・イプストウス処方）
蛇根 一銭 バレリアナ 二銭 カラムス 一銭
右を泉水一合五勺で文火にかけ少時煮る。鍋を下ろして薬が沈着するのを待って滓を去り、後方の水薬十滴を加える（水薬の処方：熱皮融液、鹿角熬煎、ナフタ・ヒドロクロール各二銭、右の三味を調合する）。
服用量は一時間毎に一食匙、さらに良質の葡萄酒一食匙を飲む。三十分毎に二回。また揮発散を併用する。その処方は

Diarrhea should be stopped quickly. In severe cases, death occurs within one or two days. Nothing works except opium. I often achieve effectiveness by using light doses of opium multiple times from the beginning and combining it with the Second Volatile Decoction. Generally, even when there is diarrhea, if the fever is intense with inflammatory swelling, the use of both opium and aconite is not permitted.
○Note by Seki: Aconite has an acrid taste and stimulates the nerves to restore paralysis, calm convulsions, and promote sweating. However, its nature is violent, and if used in large quantities, it causes paralysis and disability in people, damages blood vessels, and increases internal heat. It is best to administer small doses continuously.
Chapter 23: Treatment of Severe Cases
When the disease has progressed considerably, with rapid but weak pulse, continuous fever day and night, delirium and fumbling movements, muscle spasms and tremors, diarrhea and vomiting, this decoction should be used.
Second Volatile Decoction (Willis Ipstos Formula)
Snakeroot - 1 sen, Valeriana - 2 sen, Calamus - 1 sen
Take the above with 1 gō 5 shaku of spring water and simmer over low heat for a short time. Remove from heat, wait for the medicine to settle, remove the dregs, and add 10 drops of the following liquid medicine (Liquid medicine formula: heated bark solution, deer antler decoction, naphtha hydrochlorate - 2 sen each, mix the three ingredients evenly).
