現代語訳

弛緩して繊微の処は糜爛にも至るような者を膠固収斂し、流体の溶渙したる者もこれに斂められて濃稠になるものである。故に無毒衰弱の症に功がある。このような熱の如きは疲労があるといえども、神経が邪のために抑圧される者なれば、その害浅からざる所以なり。これは他でもない、標を治して本を棄るの理なればなり。これを譬えるに、樹木の枝梢を洗芟すれば、根がいよいよ固くなるが如く然り。 第四十一章 熱皮及び収斂剤が害をなす論 収斂補益の薬は、この病に害があって用いるべからず。もしそのキナ(熱皮)は、質が膠のようで味が渋く、胃に入って化せず、実を以て実を塡め、邪をしていよいよ固からしめ、熱をしていよいよ淫らしむ。強いてこれを用いれば、則ち数脈・下利・発熱・痞満・亡脱等の症を招くに足り、戒めざるべからざるなり。世医はあるいは謂う、熱皮は補益解熱すと。毒の有無を察せず、熱の虚実を論ぜず、吠声捕影し、認めて熱病の奇薬となし、謾投妄施し、未だかつて害なからんことなし。それ熱皮は神経衰弱・血液漏脱の人に効ありて、しかるに今これを用いて、ただに功なきのみならず、かえって害を招く者は何ぞや。蓋しその虚は真虚に非ず、邪によりて虚す。邪去れば則ち虚自ら復す。尋常の虚熱の類とは、その治を異にすべきことを以て知るべきのみ。

They consolidate and astringe what has become relaxed and deteriorated to the point of ulceration in minute areas, and even dissolved and dispersed fluids are gathered by them to become thick and viscous. Therefore, they are effective for symptoms of non-toxic weakness. However, in cases like this fever, although there is fatigue, since the nervous system is suppressed by pathogenic factors, this is why the harm is not shallow. This is nothing other than the principle of treating the manifestation while abandoning the root. To use an analogy, it is like when the branches and tips of a tree are cut away, the roots become even more solid. Chapter 41: On How Cinchona Bark and Astringent Agents Cause Harm Astringent tonifying medicines are harmful in this disease and should not be used. As for cinchona bark (fever bark), its substance is like glue and its taste is astringent. When it enters the stomach it does not transform, using substance to fill what is already substantial, causing pathogenic factors to become even more固 consolidated and heat to become even more excessive. If one forcibly uses it, it is sufficient to invite symptoms such as rapid pulse, diarrhea, fever, fullness and obstruction, collapse and exhaustion - one must be warned against this. Physicians of the world sometimes say that cinchona bark tonifies and resolves heat. They do not examine whether toxins are present or absent, do not discuss whether the heat is from deficiency or excess, bark at shadows and chase phantoms, recognizing it as a miraculous medicine for febrile diseases, carelessly and wrongly applying it - there has never been a case without harm. Now, cinchona bark is effective for people with nervous weakness and blood fluid leakage, but when used now, not only is it without benefit, but it actually invites harm - why is this? It is because this deficiency is not true deficiency, but deficiency caused by pathogenic factors. When the pathogenic factors are removed, the deficiency naturally recovers. This differs from ordinary deficiency-heat conditions, and one should know that the treatment must be different.
