富貴の家に生まれた児は、常に衣服や布団が重く暖かすぎるため、肌肉が蒸し暑くなって皮膚が脆く柔らかになるゆえに、麻疹や痘瘡が表面に発しやすいけれども、蒸し爛れた肌肉のゆえに麻疹痘瘡は重い症状となる。
貧賤の児は寒風が厳しくても衣服は薄く粗末で、温かさが少なく育つため、皮肉は堅実で壮健である。これにもまた欠点があって、麻疹痘瘡の発表がしがたく、食物が粗淡であるが、また逆に小児に不相応な膏梁（高級な肉食）を与えることがある。妊婦も飲食の節制や禁忌がなく、乳母も食事の禁止をしないため、小児の胎毒が強く、麻疹痘瘡ともに重くなる。しかしながら、貴賤を問わず、兄が重くて弟が軽いこともあり、姉が軽くて妹が重いこともあるので、先天の毒気を血に受ける時の深浅によって軽重となるものであるから、貴賤ともに医療のことは大切な事である。孝子は医を知らなければならない、という古言である。そもそも子は育て上げて父祖の業を継がせるものであるから、子孫の病気に養生を加えるのも孝の一つであるので、おろそかにすべきことではない。
小児に父母の遺毒による胎毒がある児は、麻疹痘瘡の毒が深いので、心を付けて療治すべきである。犀角を多く用いよ。
