現代語訳

波及について。『左伝』僖公二十三年に「羽毛歯革は則ち君の地より生ず。その晋国に波及する者は、君の余なり」とある。 胃が熱を下焦の気分に移し、小便不利となり、熱が膀胱に結ばれるについて。本巻の小便条に云く「熱膀胱に到れば、小便赤色となる。邪膀胱に到り、気分を干すときは、小便膠瀒(こうろう)となる」と。又云く「熱膀胱に到る者は、その邪胃に在り、胃熱下焦にある膀胱を灼く。但だ熱有りて邪無く、唯だ小便を赤色ならしむるのみ。その治は胃に在り。邪膀胱に到る者は、乃ち疫邪下焦に分布し、膀胱実にこれ邪有り、熱に止まらざるなり。胃家より来たり、治は胃に在り、兼ねて膀胱を治す。若し純ら膀胱を治せば、胃気勢いに乗じて、膀胱に擁入し、その治に非ざるなり。若し腸胃に邪無く、独り小便急数、或いは白膏馬の遺の如きは、その治膀胱に在り。猪苓湯宜し。邪気分を干する者これを宜とす」と。 熱を下焦の血分に移し、膀胱に血を蓄え、小腹硬満するについて。小便条に又云く「邪血分を干するときは、溺血蓄血となる」と。又桃仁湯方に「邪血分を干する者これを宜とす。小腹痛み、これを按ずれば硬痛し、小便自ずから調う。畜血有るなり。大黄三銭を加う。甚だしければ則ち抵当湯」とある。 癉瘧(たんぎゃく)について。瘧論に「その但だ熱して寒せざる者は、陰気先ず絶え、陽気独り発すれば、則ち少気煩寃し、手足熱して嘔せんと欲す。名づけて癉瘧と曰う。又但だ熱して寒せざるは、気内心に蔵され、而して外分肉の間に舎し、人をして消爍脱肉せしむ。故に命じて癉瘧と曰う」とある。王氷曰く「癉は熱なり」と。瘧候は下巻瘟瘧条に詳見せよ。熱の血室に入るは、下巻婦人時疫条に詳見せよ。

英語訳

Regarding "spreading influence": The Zuo Zhuan, year 23 of Duke Xi states: "Feathers, fur, teeth, and leather are produced from the ruler's land. That which spreads to the state of Jin is the ruler's surplus." Regarding the stomach transferring heat to the qi aspect of the lower burner, causing urination difficulties and heat binding in the bladder: The urination section of this volume states: "When heat reaches the bladder, urine becomes red in color. When pathogen reaches the bladder and interferes with the qi aspect, urine becomes sticky and turbid." It also states: "When heat reaches the bladder, the pathogen is in the stomach, and stomach heat scorches the bladder in the lower burner. There is only heat without pathogen, merely causing the urine to become red in color. The treatment focuses on the stomach. When pathogen reaches the bladder, this means epidemic pathogen has distributed to the lower burner, and the bladder actually contains pathogen, not limited to heat alone. Coming from the stomach system, treatment focuses on the stomach while also treating the bladder. If treating the bladder alone, stomach qi will take advantage of the momentum and crowd into the bladder, which is not the proper treatment. If there is no pathogen in the intestines and stomach, but only urgent and frequent urination, or white discharge like horse urine, the treatment focuses on the bladder. Zhuling Decoction is appropriate. This is suitable for cases where pathogen interferes with the qi aspect." Regarding transferring heat to the blood aspect of the lower burner, causing blood stasis in the bladder and lower abdominal hardness and fullness: The urination section also states: "When pathogen interferes with the blood aspect, there will be blood in urine and blood stasis." Also in the Taoren Decoction formula: "Suitable for cases where pathogen interferes with the blood aspect. Lower abdominal pain, hard and painful when pressed, normal urination - this indicates blood stasis. Add 3 qian of rhubarb. In severe cases, use Didang Decoction." Regarding dan malaria (heat-type malaria): The Malaria Theory states: "Those with only heat but no chills have yin qi cut off first, and yang qi manifesting alone, resulting in shortness of breath and vexation, hot hands and feet with desire to vomit. This is called dan malaria. Also, having only heat without chills means qi is stored internally in the heart and lodged externally between the flesh, causing people to waste away and lose flesh.
