弟子の管轄庁へ達し、その庁より当省へ添え状をもって願い出ること
一、免状を受けて開業中、事故があって廃止または死去の者は免状を差し戻すこと
一、免状並びに種痘等を渡す日は毎月三日と八日に限ること
一、種痘を受けたい者は東校並びに免許の医家へ申し出て求めること
一、免許医家は自宅において痘苗を分与する際は相当の代価を請け取って構わないこと

- Those who have received licenses and are practicing but cease operations due to incidents or death shall return their licenses
- Licenses and smallpox vaccinations shall be distributed only on the 3rd and 8th of each month
- Those who wish to receive smallpox vaccination shall apply to Toko (Eastern School) or licensed medical practitioners
- Licensed medical practitioners may charge appropriate fees when distributing smallpox vaccine at their residences
