より弥陀（みだ）観音（くわんおん）勢至（せいし）の三尊仏を授与（じゆよ）し玉ひて彼 劫病（ごうびやう）を救（すく）はせ
給ひし霊像（れいざう）なりしが其後我 日本に伝来し弥陀の尊像は信濃
国善光寺に安置し観音の像は即当寺の本尊とならせ給ふ或
時 天智天皇御 不予（ふよ）にまし〳〵けるが 帝当寺の観音 霊験（れいげん）
あらたなる事を聞し召 御祈願（こきぐわん）ありけるに御悩（ごなう）忽（たちま）ち御 平愈（へいゆ）
あらせられしかば 勅使（ちよくし）左小弁（さしやうべん）兼盛（かねもり）を以 八葉（はちよう）の宝鏡（ほうきやう）《割書：今に寺｜伝す》を
仏殿に懸（かけ）給ひ 勅願寺となし給ひぬ白鳳八年己卯又
勅宣（ちよくせん）ありて堂舎（たうしや）を建立し鳳凰山の 勅額（ちよくがく）を下し賜ふ 仁
寿三年癸酉更に一宇を造営して龍麿の像を安置《割書：後回録｜せり》【注】
す康和五年癸未 散位（さんみ）藤原 連長（つらなが）僧 智能（ちのう）共に私力（しりよく）を尽（つく）し
再営す寺僧及び下司大江 重房（しげふさ）等も亦 合力（かふりよく）して堂舎 終（つゐ）
に復古（ふくこ）せり天治元年甲辰二月朔日の地震（ぢしん）に又堂宇 破毀（はき）
す大治元年丙午春当座下司散位大江 為通（ためみち）及び長谷部（はせべ）氏

釈尊より弥陀・観音・勢至の三尊仏を授与されて、その業病を救われた霊像であったが、その後わが日本に伝来し、弥陀の尊像は信濃国善光寺に安置され、観音の像は即ち当寺の本尊となられた。ある時、天智天皇が御病気になられたが、帝が当寺の観音の霊験があらたかなることを聞し召され、御祈願されたところ、御悩みがたちまち御平癒されたので、勅使左小弁兼盛を以て八葉の宝鏡（今に寺に伝わる）を仏殿に懸けられ、勅願寺となされた。白鳳八年己卯、また勅宣があって堂舎を建立し、鳳凰山の勅額を下し賜った。仁寿三年癸酉、さらに一宇を造営して龍麿の像を安置した（後に火災にあった）。康和五年癸未、散位藤原連長と僧智能が共に私財を尽くして再営した。寺僧及び下司大江重房等もまた合力して、堂舎はついに復古された。天治元年甲辰二月朔日の地震でまた堂宇が破損した。大治元年丙午春、当座下司散位大江為通及び長谷部氏が...

Buddha bestowed the three sacred statues of Amida, Kannon, and Seishi, and these sacred images saved her from karmic illness. Later these images were transmitted to our Japan - the statue of Amida was enshrined at Zenkoji Temple in Shinano Province, while the Kannon image became the principal deity of this temple. Once, when Emperor Tenji fell ill, the Emperor heard of the miraculous efficacy of this temple's Kannon and made imperial prayers. His ailment was immediately cured, so through the imperial messenger Sashōben Kanemori, an eight-petaled treasure mirror (still preserved at the temple today) was hung in the Buddha hall, and the temple was designated as an imperial prayer temple. In Hakuhō 8, year of Tsuchinoto-U, there was another imperial decree to construct temple buildings, and the imperial plaque reading "Mount Hōō" was bestowed. In Ninju 3, year of Mizunoto-Tori, another building was constructed to enshrine a statue of Tatsumaro (later destroyed by fire). In Kōwa 5, year of Mizunoto-Hitsuji, San'i Fujiwara no Tsunanaga and monk Chinō together exhausted their private resources to rebuild it. The temple monks and the deputy Ōe no Shigefusa also joined forces, and the temple buildings were finally restored to their original state. In Tenji 1, year of Kinoe-Tatsu, on the first day of the second month, the temple buildings were again destroyed by earthquake.
