一中村勘助江戸往来して事を謀也
一近松勘六森清介と改赤穂東武に往来して事を謀功を立る
召使者にも憐愍を加へ情ある士と見へて打込候夜迄甚七と云家来
前後を見届候とかや其身福者にして甚七へも金五拾両其外形見
送りける也左右之指に手負細川殿に而外療を被仰付右之手は
療治を受左の手は頓而朽骨の身なれは療治に不及とて療治不受也
一吉田忠左衛門笹原多兵衛と改又田口一身とも改る勘六と同前功をなす
組を預り平常情有けるにや足軽寺坂吉右衛門と云者此忠左衛門
を見届とて連判に入と聞へけり
現代語訳
一、中村勘助は江戸を往来して謀議を行った。
一、近松勘六は森清介と名を改め、赤穂と江戸を往来して謀議を行い功績を立てた。召使いにも憐れみをかけ、情のある武士と見えて、討ち入りの夜まで甚七という家来が前後を見届けたということである。その身は裕福であったので、甚七へも金五十両その他形見の品を送った。左右の指に手傷を負い、細川殿のところで外科治療を命じられたが、右手は治療を受けたものの、左手は間もなく朽ち果てる身であるから治療は必要ないとして治療を受けなかった。
一、吉田忠左衛門は笹原多兵衛と改名し、また田口一身とも改名した。勘六と同様の功績をなした。組を預かり、普段から情があったのであろうか、足軽の寺坂吉右衛門という者がこの忠左衛門を見届けるとして連判に加わったと聞いている。

1. Chikamatsu Kanroku changed his name to Mori Seisuke and traveled between Akō and Edo to plan the undertaking and achieve merit. He showed compassion even to his servants and appeared to be a warrior of feeling. A retainer named Jinshichi watched over him from beginning to end until the night of the attack, it is said. Being wealthy, he sent Jinshichi fifty ryō in gold along with other keepsakes. He was wounded in the fingers of both hands, and when ordered to receive medical treatment at Lord Hosokawa's residence, he accepted treatment for his right hand but refused treatment for his left hand, saying that since he would soon be a decaying corpse, treatment was unnecessary.
1. Yoshida Chūzaemon changed his name to Sasahara Tahyōe, and also changed it to Taguchi Kazumi. He achieved the same merit as Kanroku. He commanded a group and perhaps because he was usually compassionate, an ashigaru (foot soldier) named Terasaka Kichiemon is said to have joined the pledge, claiming he would watch over this Chūzaemon.
