一 享和三年亥年六月、跡式御知行百石を相違なく下し置かれ、貞昭様常詰御小性に仰せ付けられた事
一 同月、病気が重くなったが男子がいないため、諏訪十郎左衛門の六男友之丞を養子にしたい旨を願い出て許可された事
一 同月二十五日、十五歳にて病死。法名清巌凉心。葬所は江戸渋谷長谷寺

Biological father: Second son of Shinzaemon Tōtsune
1. In the 12th month of Kansei 10, year of the horse: He devoted himself to filial piety, performed meritorious acts, studied diligently, and showed skillful abilities beyond his years. As a reward, he was granted three pieces of silver.
1. In the 6th month of Kyōwa 3, year of the boar: He was granted the hereditary stipend of 100 koku without change, and was appointed as regular attendant page (gokoshō) to Lord Sadaaki.
1. In the same month: As his illness became severe and he had no male heir, his request to adopt Tomonosuke, the sixth son of Suwa Jūrozaemon, as his heir was granted.
1. On the 25th day of the same month: He died of illness at age 15. Buddhist name: Seigan Ryōshin. Burial place: Shibuya Hasedera Temple in Edo.
