被 仰付高橋与五分組ニ而 御城御番
相勤候事
一同年八月追鳥狩之節罷出候
但此後馬ならし共ニ五度罷出内弐度
獲拝領仕候事
一同年九月当追鳥狩之義両度之狩
共ニ隊々連属都合之次第宜相見且列
伍足並進退等宜相揃畢竟致出精候
義と被 思召之旨御称美被成下候
但此後毎度御称美被成下候事
一文化三寅年三月久五郎と改名仕
度旨被任願候事
一同四卯年二月御免山〆り方勤被
仰付候事

現代語訳
一、享和三年亥年三月、親の願いの通り御番代を
仰せ付けられ、高橋与五分組にて御城の御番を
勤めた
一、同年八月、追鳥狩の節に出向いた
ただし、この後馬ならしとともに五度出向き、内二度
獲物を拝領した
一、同年九月、当追鳥狩について、両度の狩とも
隊々の連属や都合の次第がよく見え、かつ列
伍足並み進退等がよく揃い、畢竟出精した
ということを思し召しの旨、御称美を成し下された
ただし、この後毎度御称美を成し下された
一、文化三年寅年三月、久五郎と改名したい
旨、願いの通り任せられた
一、同四年卯年二月、御免山締り方勤めを
仰せ付けられた
一、同五年辰年十二月、御免山締り方勤めを
仰せ付けられ、出役したことについては格別の締りにも
なり、臨時の御用を出精して勤めたため、御酒を下し置かれた
