一、宝暦12年午年2月 頭の梁瀬求馬が御役替えになったため、跡御役の山崎左助組となった事
一、明和4年亥年12月 願いの通り息子伴助を御番代に仰せ付けられた事
一、安永4年未年10月 息子伴助が病死したため、出勤すべきところを病気により願い出て、小普請料を差し上げて罷り在った事
一、同年12月 願いの通り次男直右衛門を嫡子にしたい旨、願いをお任せいただいた事
一、同年閏12月 願いの通り息子直右衛門を御番代に仰せ付けられた事
一、天明5年巳年5月3日 72歳にて病死。法名円亮院一参治■、葬所同上
