現代語訳
一、天和2年戊年正月 車市郎左衛門をお預かりしていたが、同年6月追放となったため須川野口へ送り届けて帰参した事
一、同3年亥年9月 勤番のため江戸表へ上り、翌子年4月に帰参した事
一、貞享元年子年8月 土方伊賀守殿の警固を務め、当町から赤谷口まで赴いた事
一、同2年丑年（月は不詳） 役儀を免じて頂きたい旨を訴訟申し上げたところ、時節をもって免許するが、それまでは職務を続けるよう仰せ出された事
一、元禄元年辰年2月 再度の願いにより役儀を免じられ、無役組を仰せ付けられた事
一、同年6月 御入部の節に江戸へ上り、御小性に同道して帰参した事
一、同年9月 願いの通り息子三郎左衛門が御番代を仰せ付けられた事
一、同6年酉年5月 願いの通り隠居を仰せ付けられた事
一、同12年卯年3月26日 76歳にて病死、法名洞仙院殿白翁惟三、葬所は同上
