笑ひ候節歯くき出候方
一歯並揃ひ色白き方
一百会より後ろ江懸壱寸程之古切疵
三ケ所有之
一月代常体右之鬢薄き方
一左之足向すねよりふくら脛江懸け
巾壱寸【一字抹消】竪八寸程之古切疵跡有之
一其節之衣類木綿藍竪縞羽織を
着し御納戸色股引をはき紺茶竪
縞小倉帯を〆罷在
右之通之もの於有之は其処に留置御料は
御代官私領は領主地頭へ【可脱ヵ】申出夫ゟ江戸に
おゐて鍋嶋内匠頭番処へ可申出候若及見
聞候はゝ其段も可申出候尤家来又者等迄入
念可遂吟味候若隠し置脇よる（より）相知候はゝ可
為曲事候
現代語訳
笑う時に歯茎が出る方
一、歯並びは揃っており色は白い方
一、百会より後ろへかけて一寸程の古い切り傷が
三ヶ所ある
一、月代は普通で右の鬢が薄い方
一、左の足の向こう脛からふくらはぎにかけて
幅一寸縦八寸程の古い切り傷跡がある
一、その時の衣類は木綿の藍縦縞羽織を
着して御納戸色の股引を履き紺茶縦
縞小倉帯を締めていた
右の通りの者がいる場合は、その場所に留め置き、御料地は
御代官へ、私領は領主・地頭へ申し出て、それから江戸において
鍋島内匠頭の番所へ申し出ること。もし見聞きした場合は
その旨も申し出ること。もちろん家来や者共まで入念に
吟味を遂げること。もし隠し置いて他から知れた場合は
曲事とする。
