　　　処於瀧沢御紋御羽織被下置候事
　一　同年五月精出相勤候ニ付御紋御帷

　　　精相勤候ニ付金五百疋被下置候事
　一　同年十二月親昌慶願之通隠居被
　　　仰付家督御知行百石無御相違被下
　　　置御番医被　仰付只今迄被下候稽
　　　古料五人扶持ハ親隠居料ニ御振替

　一　寛保元《割書：酉》年五月御人払ニ而隠密之
　　　御療治被　仰付早速御平愈被遊
　　　候ニ付為御褒美純子三巻被下置候

現代語訳

処で瀧沢において御紋御羽織を下し置かれ候事

一、同年五月　精出し相勤め候につき御紋御帷子一つを下し置かれ候事

一、同年六月　江戸表への御暇を下し置かれ、出精相勤め候につき金五百疋を下し置かれ候事

一、同年十二月　親昌慶の願いの通り隠居を仰せ付けられ、家督御知行百石を相違なく下し置かれ、御番医を仰せ付けられ、只今まで下され候稽古料五人扶持は親の隠居料に御振替下し置かれ候事

一、寛保元年酉年五月　御人払いにて隠密の御療治を仰せ付けられ、早速御平癒遊ばされ候につき、御褒美として純子三巻を下し置かれ候事

一、同年七月　御納戸払いの御品を下し置かれ候
　但しこの後隔年に下し置かれ候処、御品は不詳に候事

一、同年八月　出精相勤め候につき御側医師を仰せ付けられ、薬種料として金年々十両ずつを下し置かれ候事

英語訳

At Takizawa, granted a silk haori bearing the lord's crest

First entry: Same year [Genbun 4], 5th month - Due to diligent service, granted one silk summer garment bearing the lord's crest

First entry: Same year, 6th month - Granted leave to Edo, and due to excellent service, awarded 500 mon in gold

First entry: Same year, 12th month - As requested by his father Shōkei, granted permission to retire. The family headship and stipend of 100 koku were transferred without change. Appointed as attending physician (go-ban'i). The training allowance of provisions for 5 retainers previously granted was transferred to become his father's retirement allowance

First entry: Kanpō 1, Year of the Rooster (1741), 5th month - Appointed to provide secret medical treatment in private audience. Due to prompt recovery, granted three rolls of pure silk as reward

First entry: Same year, 7th month - Granted items from the lord's personal storehouse
　Note: Thereafter granted [such items] every other year, but the specific items are unknown

First entry: Same year, 8th month - Due to excellent service, appointed as personal physician (go-sokui-ishi). Granted 10 ryō annually for medicinal materials
