現代語訳

物相煩い候に付き療治仕り候ところ段々平愈仕り候に付き、同七月罷り下り、御上京御供は仕らず候事

一、七月同年江戸において、是まで之御擬作御知行百五十石に成し下され、其の上御羽織並びに六歌仙下し置かれ候事

一、元禄七年戌年閏五月　御迎えとして立ち帰り罷り登り候事

一、同年七月　丹羽左京亮様より仰せ遣わされ候に付き二本松へ罷り越し、同八月罷り帰り候事

一、同十年丑年六月　願いの通り忰昌慶御番代仰せ付けられ候事

一、宝永五年子年二月六日　八十四歳にて病死、法名清誉壽心信士、葬所

Due to falling ill, underwent medical treatment and gradually recovered completely, so returned in the 7th month of the same year and did not serve in accompanying the lord to Kyoto
