一侍中番外以上他出之節肩衣掛け罷出候筈幷侍中無僕にて罷出候面々

一侍中痛所有之町方薬湯へ幾廻り入湯仕度願之儀も先格有之尤願口上書
指出す事医師指図之訳も認候事毎日通ひ申事に候得は初て此方へ相達
其後今日切に入湯相仕廻候段都合前後断に相帰候事

現代語訳
一、侍中番外以上が他出の節は肩衣を着けて外出する筈。並びに侍中で供を連れずに外出する面々は御紋装束をしない筈
一、侍中で痛む所があり町方薬湯へ幾度も入湯したい願いの儀も先例がある。もっとも願い口上書を差し出す事。医師の指図の理由も認める事。毎日通う事である場合は初めてこちらへ相談し、その後今日限りで入湯を仕舞う段、都合前後の許可に帰す事

1. When samurai of ban-gai rank and above go out, they shall wear shoulder garments (kataginu). Also, samurai who go out without attendants shall not wear crested formal attire.
1. Regarding requests by samurai who have ailments and wish to visit town medicinal baths repeatedly, there are precedents for this. They must submit a written petition. The reason for the physician's instructions must also be acknowledged. When it involves daily visits, consultation with this office is required initially, and afterward, when the bathing treatment is completed for the day, permission is returned with notification of the circumstances.
