江戸へ入れ置かれ候旨仰せ付けられ候事

一、同十二年午年五月五日病死、法名信行院清林日明居士、葬所武州三田西蓮寺

　実は大門瀬五右衛門貞知の嫡男
　妻は秋山文右衛門政賀の女、享保十五年戌年誕生、文化五年辰年八月七日病死、法名演暢院妙清大姉、導師長照山久福寺、葬所白龍山融通寺

一、宝暦八年寅年（月不詳）親の病気の内、代番勤めを仰せ付けられ候事

一、同十二年午年（月不詳）親の御擬作三両二人扶持を下し置かれ、江戸常詰甲賀の者に召し出され候事

一、同十三年未年八月、会津勝手を仰せ付けられ候事

一、同年十二月、御台所家具役仮役を仰せ付けられ候事

一、明和五年子年十一月、吟味所定番を仰せ付けられ候事
