死ニ付跡御役山崎左助組ニ罷成候事

召放候ニ付跡御役梶原左門組ニ罷成候事
一 同五丑年九月追鳥狩之節罷出候
但以後三度罷出候事
一 同六寅年四月去追鳥狩之節足並
列伍之次第宜相揃兼而内稽古等出
精致候故之義と被 思召之旨相組
一同御称美被成下候

現代語訳
替わりにより、跡の御役で杉田五郎兵衛組に所属することになった。
一 天明元年丑年九月、頭の杉田五郎兵衛が病死したため、跡の御役で山崎左助組に所属することになった。
一 寛政元年酉年九月、頭の山崎左助が御役を召し放たれたため、跡の御役で梶原左門組に所属することになった。
一 寛政五年丑年九月、追鳥狩の節に出陣した。
但し、以後三度出陣した。
一 寛政六年寅年四月、先の追鳥狩の節に、足並みや列伍の次第がよく整い、兼ねて内稽古等を精進していたことによるものと思し召しの旨で、相組一同が御称美を賜った。
但し、この後も毎度御賞美を賜った。
一 寛政十二年申年二月、息子の新十郎が不慎の所行があったため、退嫡を仰せ付けられ、二男の熊五郎を嫡子にしたい旨を願い出て認められた。
一 文化四年卯年九月、馬揃えの節に出陣し、一夜宿陣をした。
一 同年十一月、蝦夷地御固の御用を仰せ付けられたところ、六十歳以上の者は出陣の御用を免除されるが、息子がいる者は陣代を差し出すようにとの仰せ出があったため、願い出て
